QUALIFICATION
The Search is open to incumbent AFC Chairpersons who have rendered at least three (3) consecutive years of service as Chairperson and at least five (5) consecutive years of service for Secretariat-Coordinator.

CRITERIA FOR EVALUATION

AFC CHAIRPERSON

  1. PERFORMANCE AND IMPACT (60 pts.)
  2. LEADERSHIP (25 pts.)
  3. INTERVIEW (15 pts.)

AFC SECRETARIAT- COORDINATOR

  1. PERFORMANCE AND OUTPUT (50 pts.)
  2. CREATIVITY, INNOVATIVENESS AND RESOURCEFULNESS (40 pts.)
  3. SUSTAINED COMMITMENT AND SERVICE TO THE AFC (5 Pts.)
  4. INTERVIEW (5 pts.)

EVALUATION PROCEDURE

A. Outstanding MAFC/CAFC Chairperson and Secretariat-Coordinator

  1. The Chairperson and the Secretariat-Coordinator maybe nominated by the MAFC, CAFC, PAFC, HUCAFC, ICCAFC member/s, Municipal Government or by any institution or member of the community, and shall be endorsed by the Local Chief Executive or Municipal/City Agriculturist/Head, Agriculture/Veterinary Office using the attached Nomination Form.
  2. A scrapbook shall be prepared following the sequence in the attached evaluation criteria with corresponding documentary requirements.
  3. The scrapbook shall be submitted to the Office of the Provincial Agriculturist.
  4. The Office of the Provincial Agriculturist shall create a screening committee to be composed of the following:

  5. 4.1 Head Provincial Agriculture Office or Provincial Veterinary Office
    4.2 PAFC, HUCAFC, ICCAFC Chairperson
    4.3 Department of Interior and Local Government Representative
    4.4 Representative of an existing DA accredited NGO in the province
    4.5 Representative of the State Universities and Colleges or academe

    • The Provincial AFC Secretariat-Coordinator shall act as secretariat during the deliberation for both the MAFC, CAFC Chairperson and MAFC, CAFC Secretariat-Coordinator.

  6. The selected nominee shall be endorsed to the RAFC and to the DA-RFO by the Provincial/City Agriculturist/Head, Agriculture/Veterinary Office.
  7. The RAFC and the DA-RFO shall create an Evaluation Committee composed of the following:
    1. DA-RFO Regional Technical Director
    2. NEDA-Regional Development Council representative
    3. Representative from the academe or an existing DA accredited NGO in the region.
    4. Department of Interior and Local Government (DILG) Regional Representative
      • The RAFC Secretariat-Coordinator and the staff shall act as secretariat during the deliberation.
  8. The Evaluation Committee shall:
    1. Come up with the Regional Outstanding MAFC, CAFC Chairperson and Secretariat-Coordinator; and
    2. Endorse them to PCAF for the national Search for Outstanding MAFC, CAFC Chairperson and Secretariat-Coordinator.

B. Outstanding PAFC, HUCAFCC, ICCAFC Chairperson and Secretariat-Coordinator

  1. The Chairperson and the Secretariat-Coordinator maybe nominated by the MAFC, CAFC or HUCAFC, ICAFC, PAFC member/s, Provincial Government or by any institution or member of the community and shall be endorsed by the Local Chief Executive or Provincial/City Agriculturist/Head, Agriculture/Veterinary Office, using the attached Nomination Form.
  2. A scrapbook shall be prepared following the attached evaluation criteria with the corresponding documentary requirements.
  3. The scrapbook shall be submitted to the Department of Agriculture-RFO.
  4. The Evaluation Committee created at the regional level shall screen the nominee and endorse to PCAF the regional nominees for the PAFC, HUCAFC, ICCAFC Chairperson and Secretariat-Coordinator.
  5. The PCAF shall create a National Technical Committee (NTC) who shall evaluate the regional nominees. The Committee shall be composed of representatives from the following:
    1. Department of Interior and Local Government-Bureau of Local Government Supervision (DILG-BLGS)
    2. Philippine National Volunteer Service Coordinating Agency (PNVSCA)
    3. National Dairy Authority (NDA)
    4. Philippine Statistics Authority (PSA)
    5. Philippine Council for Agriculture and Fisheries (PCAF)
  6. Board of Judges (BOJ) shall be created and convened to come up with a national winner as Outstanding PAFC, HUCAFC, ICCACF Chairperson and Secretariat-Coordinator. The BOJ maybe composed of representatives from the private sector. 
  • PDD-VMS staff shall act as secretariat during the deliberation.

C. Outstanding RAFC Chairperson and Secretariat-Coordinator

  1. The Chairperson and the Secretariat-Coordinator may be nominated by the MAFC, CAFC, HUCAFC, ICAFC, PAFC member/s, Local Government Units or by any institution or member of the community, using the attached Nomination Form.
  2. A scrapbook shall be prepared following the attached evaluation criteria with the corresponding documentary requirements.
  3. The scrapbook shall be submitted to PCAF.
  4. The National Technical Committee (NTC) shall be composed of representatives from the following:

    1. DILG-BLGS
    2. PNVSCA
    3. NDA
    4. PSA
    5. PCAF
  5. The NTC shall come up with the top three finalists and validate them for this category
  6. The Board of Judges shall be convened to come up with a national winner as Outstanding RAFC Chairperson and Secretariat-Coordinator. The BOJ shall be composed of representatives from the private sector.  
    • The PDD-VMS staff shall act as secretariat during the deliberation.

I. SEARCH RULES & REGULATIONS

  1. A performance period of two years preceding the start of the Search will be covered for evaluation: for AFC Chairperson – from July 31 to June 30 of the two years rating period while for the Secretariat-Coordinator – January to December of the two years rating period.
  2. One (1) regional winner or entry for each level and category (RAFC; PAFC, HUCAFC, ICCAFC, and MAFC, CAFC) shall be properly endorsed by the DA-RFO, and shall be considered for national evaluation.
  3. The decision of the Board of Judges shall be final and executory.
  4. The national winner’s scrapbook and other documents submitted for evaluation shall become the property of PCAF.
  5. A national winner shall not be eligible for nomination in the same category in the Annual Search within two (2) years after winning the award.
  6. Scrapbook presentation must be arranged according to the sequence of set criteria.

II. DOCUMENTARY REQUIREMENTS

  1. Duly filled out AFC Membership Profile (VMS FORM 2- Revised 2018) and nomination forms (Form 1.A, 1.B, 1.C) with two (2) latest passport size pictures.
  2. Organizational records (current organization memberships).
  3. All documents and materials indicating specific meritorious contributions, accomplishments in the field of endeavor.
  4. Other accomplishments including Citations and Awards (photocopies of certificate/s, duly certified).
  5. Related photographs with explanatory captions.
  6. Fact sheet which contains the justification for the nomination and/or outstanding/notable accomplishments during the rating period.
  7. Copy of regional score sheet.
